A diagnosis of Alzheimer's, vascular dementia, Lewy body or frontotemporal dementia changes everything — but it doesn't have to mean leaving home. Dementia care that may cost your family nothing

Medicare's Guiding an Improved Dementia Experience — the GUIDE Model — is a federal program that funds the kind of in-home memory care families usually pay for themselves. Through our CMS-approved partners, eligible Medicare beneficiaries can receive up to 72 hours of complimentary in-home dementia care each program year (July 1 – June 30), along with care coordination and family resources at no out-of-pocket cost.

What is in-home memory care?

In-home memory care is one-on-one support for people living with Alzheimer's or another form of dementia, delivered at home by a caregiver trained in dementia care. Your loved one gets help with daily tasks, personal care, safety and engagement — without a move to a facility — on a schedule that works for your family.

Can someone with Alzheimer's or dementia safely stay at home?

Very often, yes. With the right plan, many people with dementia age in place safely. Our caregivers reduce fall risks, manage medications, supervise higher-risk activities like cooking, and watch for changes in condition — keeping home structured, predictable and secure.

Sundowning is a pattern of late-day confusion, agitation or restlessness common in dementia. Our caregivers respond by dimming lights, lowering noise, redirecting to comforting activities and keeping evening routines steady — so your loved one feels settled as the day winds down.
