How to choose a home care agency you can trust

Not all home care agencies are the same, and the difference shows up exactly when you need it most. Here are the questions worth asking before you hire — and what the answers should sound like.

Choosing a home care agency usually happens under pressure, after a fall, a hospital discharge, or a diagnosis that changes everything. That's exactly when it's easiest to skip questions that matter. Before you sign anything, the National Institute on Aging recommends confirming a handful of things: whether the agency is properly licensed, how caregivers are screened and trained, whether you'll see the same caregiver consistently, what happens if that caregiver is unavailable, how the agency communicates with your family, and whether pricing is transparent with no hidden fees.

Every reputable agency should be able to answer these clearly and in writing. If an answer is vague, evasive, or "we'll figure that out," that's useful information too. Below is what to ask, why it matters, and what a good answer actually sounds like.

Is the agency licensed, bonded and insured?

This is the baseline, not a bonus. A licensed, bonded and insured agency protects your family from liability if something goes wrong, a very different situation from hiring a caregiver privately, where you'd be the one responsible for payroll taxes, workers' compensation and any coverage gap if that caregiver doesn't show up.

How are caregivers screened and trained?

Ask specifically about criminal background checks, reference verification and what training caregivers complete before they're placed with a client. A good agency won't just say "we check everyone." They'll describe the actual process.

Will we see the same caregiver, and what happens if they're unavailable?

Consistency matters, especially for someone living with memory loss or anxiety about new people. Ask how caregivers are matched to your loved one's needs and personality, and, just as important, what the backup plan is if your regular caregiver is sick or unavailable. A shrug is a red flag; a clear answer is a good sign.

How will the agency communicate with our family?

You should never have to wonder how your parent's day went. Ask how often you'll hear updates, who to contact with a concern, and whether that's a real person or a call center. Family involvement in the care plan, not just notification after the fact, is what separates a good agency from an adequate one.

Is the pricing transparent — and is there a long-term contract?

Ask for costs in writing before service starts, and ask directly whether there are fees beyond the hourly rate: mileage, minimum-hour requirements, cancellation penalties. Just as importantly, ask whether you're locked into a contract. Care needs change; your agreement should be able to change with them, not against you. What if the caregiver isn't the right fit?

Even a well-matched caregiver isn't always the right personality fit, and that's normal. Ask how easily you can request a change. It should be simple, not something you have to fight for.

A quick checklist to bring to your first call

  • Are you licensed, bonded and insured?
  • How do you screen and train your caregivers?
  • Will it be the same caregiver each visit, and what's your backup plan?
  • How and how often will you communicate with our family?
  • What's included in your rate, and is anything billed separately?
  • Is there a minimum number of hours, or a long-term contract?
  • How quickly could care actually start?
